Apple Health medication alternatives
Before replacing Apple Health Medications, check whether you have turned on Critical Alerts. Most people looking for an alternative are looking because the reminders are not reaching them, and Apple has a setting for exactly that which almost nobody finds.
Open Health, tap Medications, then Options, and turn on Follow Up Reminders. You can then enable Critical Alerts per medication. Apple’s documentation says these appear on the Lock Screen and play a sound even when a Focus is on or the iPhone is muted. It costs nothing and it may be the whole fix.
Apple Health also does more than it is given credit for: as-needed doses, an Apple Watch app, PDF export of your medication list, Siri logging, and drug interaction warnings in the United States.
If one follow-up is not enough
Apple Health follows up once, 30 minutes after a missed dose, and then stops. If you reliably sleep through or dismiss both, you need something that does not give up.

Apple Health can share your medication list with family through Health Sharing, but it is a shared view rather than a missed-dose alert.
